Please Read this Agreement Carefully:

  • I will start and end my event at stated time.
  • My group will not smoke or have tobacco or alcoholic beverages on school grounds, at my event.
  • Adults will supervise children at ALL TIMES.
  • I am responsible for damages and cleanup.
  • I agree that there will be no firearm or weapons allowed on campus unless pre-approved as part of my program.
  • I agree that any school closure due to holidays, vacation, bad weather or other emergency, cancels my use of facilities without notification.
  • I agree that failure to observe rules and regulations of the District may result in loss of usage privileges.
  • I agree that my group will park in designated parking area and not in fire and emergency vehicles lanes.
  • I agree to give 24 hour notice of cancellation.
  • I agree to use only the equipment listed on the application, and to stay only in the area of the school that I requested.
  • I understand that School District functions preempt all other usage.
  • I understand, in case of fire alarm that my group will EXIT building immediately and call 911.

Agreement: I hereby make application to the Woodland School District for use of school facilities described above, and certify that the information given in this application is correct. I further state that I have the authority to make this agreement on behalf of the above organization, or for my individual group, and agree that all members of this group will observe all rules and regulations (specifically Policy and Procedure No. 4260-Use of School Facilities) of the Board of Directors and Administration of any Woodland School District facilities that are used. I am 18 years of age or older, and agree to defend, indemnify and hold harmless the Woodland School District, its School Board, Administrators, Employees, Volunteers and Agents, from any and all claims, injuries, damages, causes of action, losses, costs (including attorney's fees and costs) suits or judgments due to bodily injuries, personal injuries and/or property damage arising directly or indirectly out of the use of the District's facilities for the activity noted above, except to the extent such injury or damage is caused by or results from the District's own negligence in connection with this agreement.
